On receiving your enquiry by telephone, email or letter a member of our management team will contact you to discuss your enquiry in more detail. We will then arrange to come and meet you at your home to carry out an assessment of you needs.

The assessment process is carried out at your home. A member of our management team will visit you to get to know you. They will carry out a number of assessments to gather information and understand your home life situation. Following the assessment visit a tailored package of care will be offered to you.

Care Plan
Once you are happy with the plan of care we will finalise this with you and your family and make sure we have all of the necessary information to fully meet your needs. If necessary, we may identify suitable health professionals or other services to support you further.

Service Commences
Your service will commence on the agreed date. Our Manager will check in with you after the first week to ensure that your service is running smoothly and that you are happy.

Service Reviews
We hold review meeting with you after 6 weeks of your service starting and then every six months. These meetings are held to ensure that we are still meeting your needs and to identify any changes. If anything needs to be updated, we will work with you to make the changes you need.
